TEMPLATES
The below templates are meant to help you with the internal structure of your Ukrainian Student Organization (USO). It’s difficult when starting up a USO and not having a base to start with, so hopefully this will ease with the transition, amend these forms to the specific needs of your USO.
This is the comprehensive constitution from the Ukrainian Students’ Society in Edmonton; use it as a template to formulate a constitution for your local Ukrainian Student Organization.
USO INDIVIDUAL MEMBERSHIP FORM – TEMPLATE
This form is meant to help form a membership list to help keep records of the individual membership of your Ukrainian Student Organization.
EXECUTIVE CONTACT LIST – TEMPLATE
This template is meant for the Executive’s information and is to be sent out so that everyone has each other’s contact information.
USO EXECUTIVE MEETING MINUTES – TEMPLATE
This template is meant to help keep records of your Ukrainian Student Organization’s Executive Meetings. This is important so that should any discrepancies arise in the future; there is something to refer back to.
EXPENSE CLAIMS FORM – TEMPLATE
This template is meant to help your Ukrainian Student Organization be financially accountable. This helps with financial transparency and is crucial to have when applying for grants.
LIVE PERFORMANCE AGREEMENT – TEMPLATE
This template can be altered for your individual needs when hiring a band for a performance. This is important in order to help protect all parties involved. It is also important to note that some bands do not have business bank accounts and cannot provide receipts for any transactions. Therefore, when applying for grants this ‘Live Performance Agreement’ may suffice as proof of what the money is being spent on when filing out a report for a grant. Remember the deal is worth as much as the paper it’s written on!
